Service Terms:
- An agreement for this membership will be presented in person or via e-signature in order for membership to begin.
- All included services rendered in-office.
- Appointment required.
- Upgrades may be added at the time of service for an additional fee.
- Additional units will be available at the time of service for a discounted rate.
- We reserve the right to refuse membership to anyone at our discretion.
- We reserve the right to change the terms of this membership at any time without notice.
- Member discounts are not combinable with other promotional discounts.
- Reward points may not be combinable with member discounts.
- An appointment revision or cancellation that is not per our policy, or not showing up for a perk appointment, results in forfeiture of that perk.
* We can send the agreement electronically or it may be signed in person.
Cancellation Terms:
- Cancel by phone, email or in writing.
- 30-Day Notice to Cancel.
- Fee Charged unless term completed.
- Membership Fees are Non-Refundable.
- Minimum of 12 months, and month to month after.
- This membership fee recurs monthly unless canceled prior to the end of the term.
- If membership is canceled, the dollar value of sum total of benefits is to be paid.
